A member of our admissions team will follow up promptly to review needs, gather additional clinical detail if required, and coordinate care.

Who Can Make a Referral?

Hospice referrals may be submitted by:

  • Physicians and medical providers

  • Hospital discharge planners

  • Case managers and social workers

  • Skilled nursing and assisted living communities

  • Family members or primary caregivers

  • Patients themselves (when appropriate)

No physician order is required to start the referral process, and we can assist in obtaining one as needed.

Hospice Eligibility

Patients may qualify for hospice when:

  • They have a life-limiting illness with a prognosis of six months or less

  • Curative treatment is no longer the primary focus

  • Comfort, dignity, and quality of life are the priorities

  • Functional decline, symptom burden, or increasing care needs are present
